IT Service Request Forms - NEW

We have implemented a new IT Service Request portal. To log directly into the Service Request system to see all available requests, visit http://hsm.ccsu.edu and log in with your BlueNet username and password (you may need to prefix your username with ccsu_comp_srv\ - for example, ccsu_comp_srv\ab1234).

To request any of the items/services listed below, please click on the link and enter your BlueNet username and password if prompted. Complete the online form and click on Review &Submit; review the information and click on Submit. You will receive an email indicating your request has been received.
